Club OS can build a standalone outreach form that can be used on any device during outreach/community events. Each submission will auto-create a prospect profile. The origin will be ‘outreach’. The marketing source will be selected on the form. The following preferences need to be provided:

• Font Color
• Title and Description

  • Recommended Description: Fill out this form to receive more information about [Club-name]
• Fields (Data to Capture)
• Fields in Club OS the captured data will map to
• Fields that should are required
• ‘Submit Button’ options: Title, Front, Front Color, Button Color
